Transference-Focused Psychotherapy (TFP) is a structured, step-by-step approach to treating severe personality disorders, particularly borderline and narcissistic. Its goal is to foster lasting personality change, reflected in improved functioning, greater satisfaction in relationships and work, and reduced symptoms. Rooted in psychodynamic theory, TFP draws on the object relations model of mental functioning. The core strategy involves transforming internalized patterns of relating by examining how they emerge in the therapeutic relationship (through transference) and in the patient’s everyday interactions. TFP focuses on deeply ingrained images of the self and others that shape interpersonal behavior. Exaggerated, distorted, and unrealistic internal representations—common in severe personality disorders—are linked to mood instability, fragile self-esteem, and troubled relationships.

For example, if a parent frequently yelled at you as a child, your mind internalized both roles: the frightened child and the aggressive parent. In adulthood, certain triggers activate these dyads, causing people to shift between different versions of themselves. TFP emphasizes that these shifts often occur outside conscious awareness, particularly regarding the aggressive side of the dyad. Therapy focuses on bringing these hidden structures into conscious recognition. TFP stands out among psychotherapy methods due to its strong empirical foundation and proven effectiveness in treating borderline personality disorder. Meaningful change becomes possible only when individuals acknowledge, accept, and transform the aggressive aspects of their nature.
